NEW PATIENTS

WHAT TO EXPECT ON YOUR FIRST VISIT

Patients visiting Ability Rehab and Foot Clinic  for the first time will be required to fill out a Patient Intake Form and Consent Form.  Your practitioner needs to know important health-related information before he or she can provide you with the best treatment plan. This also gives you, the patient, an opportunity to get to know your practitioner better.

New Patients are requested to come 15 minutes prior to their scheduled appointment in order to fill out the forms.  You can also save time by printing the patient forms online, and filling them out before-hand.  

 

Please remember to bring these forms with you as well as any referrals, medication lists and pertinent medical reports.
